Job Description

We are looking to recruit an enthusiastic qualified Advanced Clinical Practitioner/Advanced Nurse Practitioner with prescribing to come and join our lovely team in Bradford City. We offersupport and mentorship as well as the right level of autonomy. We have been awarded Gold award Investors In People, CQC Good rating and we are high QOF achievers. We offer opportunities for development and special interests and are looking for someone who will fit into our current team who work closely together with our current team of practitioners and with our Salaried GP's.

Main duties of the job

To deliver a high standard of patient care as anAdvanced Nurse Practitioner (ANP)/Advanced Clinical Practitioner in general practice, using advancedautonomous clinical skills, and a broad and in-depth theoretical knowledge base

Telephone triage, face to face clinics, pathologymanagement and development of team and peer support

Prescribing efficiently and effectively within boundaries

To manage a clinical caseload, dealing withpresenting patients needs in a primary care setting

To provide clinical leadership within the practice.

To manage own clinical workload in general practiceresponding effectively to patient need and ensuring patient choice and ease ofaccess to services.

To be available to mentor and support other nursesand health care assistants in developing and maintaining clinical skills

Weare also a training practice for Trainee Nurses.

We are consistently high QOF achievers, CQC Good(July 2022), Investors in People Gold Award achiever's (December 2022 and reaccredited in 2025) andparticipate in numerous Enhanced Services including Anticoagulation, Diabetes,Ultrasound services and Dermatology.

Job responsibilities
  • To work in partnership with the PrimaryHealth Care team (PHCT) in providing patient-focussed care responsive tohealth need.
  • To work in partnership with the PHCT indeveloping new services for patients.
  • To utilise the skills / knowledge andcompetencies required in order to provide first point of contact directaccess for patients with undifferentiated and undiagnosed problems seekinga primary health care service.
  • To utilise the skills / knowledge andcompetencies required in order to be responsible and accountable forassessing patients with undifferentiated and undiagnosed health careneeds.
  • To utilise skills / knowledge andcompetencies required in order to be responsible and accountable formanaging a caseload of patients, for treating referring, admitting anddischarging patients as appropriate following training.
  • To utilise skills / knowledge to makejudgments involving complex facts or situations, which require analysis,interpretation and comparison of a range of options, which could becomplex and made up of several conflicting components and where expertopinion may differ or not all information is available.
  • To utilise skills / knowledge andcompetencies required in order to be responsible and accountable formanaging the nursing team.
  • To utilise skills / knowledge andcompetencies required in order to be responsible and accountable forachievement of Quality and Outcome Framework (nGMS GP contract) targetsset by Avicenna Medical Practice.

Qualifications

MSc in Advance Practice, Current NMC/HPC/Pharm Registration, Registered prescriber, Understanding of NHS Plan/NSFs and implications for Primary Care Evidence ofhaving training in clinicalexamination, history taking, therapeutic communication, pharmacology,pathophysiology etc. Evidence of continuing professional development Minimumof 2 years post-registration experience of working in primary / community /acute health care setting Minimum of 1 years experience of managing the careof patients in the primary care setting Experience of providing effectivehealth education and health promotion to patients presenting with a range ofhealth needs, Experience of developing evidence basedprotocols, Experience of instigating investigations, interpretinginvestigations and initiating appropriate plan of care for patients.Experience of assessing patients with undifferentiated diagnosis. Assessmentskills in history taking and clinical examination
