Location: Walk-In Centre / Urgent Treatment Centre
Shift Pattern: 12 hour shifts (Days, Nights, Weekends, and Bank Holidays)
Contract: Flexible / Bank / Permanent (Shifts available all year round)
Reports to: Clinical Lead / Service Manager
The Advanced Clinical Practitioner (ACP) will provide autonomous, high quality clinical care to patients presenting with minor illness and urgent care needs within the Walk In Centre. The post holder will independently assess, diagnose, treat, and prescribe medications within scope of practice.
The ACP will utilise advanced clinical decision making skills and work autonomously while supporting the wider multidisciplinary team. Accurate documentation using electronic clinical systems including SystmOne and Adastra is essential.
The ACP will be educated to Level 7 (MSc Advanced Clinical Practice) or equivalent, and hold an independent prescribing qualification.
Key Responsibilities Advanced Clinical Assessment and ManagementIndependently assess, diagnose, and manage patients presenting with minor illness and undifferentiated conditions.
Undertake comprehensive history taking, physical examination, and clinical assessment.
Formulate differential diagnoses and implement appropriate treatment plans.
Safely manage a wide range of minor illness presentations including acute infections, respiratory conditions, dermatological presentations, and other urgent care presentations.
Recognise red flags and elevate patients requiring higher levels of care.
Prescribe medications independently in accordance with clinical guidelines and scope of practice.
Safely prescribe for minor illnesses using evidence based practice.
Review current medications and ensure safe prescribing practices.
Provide patients with medication advice and safety information.
Work autonomously as an advanced practitioner within professional scope of practice.
Make complex clinical decisions based on assessment findings.
Refer patients appropriately to GP, Emergency Department, or specialist services where required.
Provide advice, treatment, and discharge planning.
Accurately record patient consultations, assessments, treatment plans, and prescriptions using SystmOne and Adastra.
Maintain clear, accurate, and legally compliant clinical records.
Ensure compliance with information governance and confidentiality policies.
Deliver safe, effective, and patient centred care.
Work within local and national clinical guidelines, protocols, and policies.
Identify and manage clinical risk appropriately.
Participate in clinical governance, audit, and quality improvement initiatives.
Adhere to safeguarding policies and procedures.
Communicate effectively with patients, relatives, and carers.
Work collaboratively within a multidisciplinary team including GPs, ACPs, nurses, paramedics, and administrative staff.
Provide clinical support and guidance to junior staff when appropriate.
Ensure effective handover and continuity of care.
Maintain professional registration with NMC or HCPC.
Maintain independent prescribing registration.
Practice in accordance with professional codes of conduct.

Qualifications and Registration
Registered Nurse (NMC) or Registered Paramedic (HCPC) or Allied Health Professional
MSc Advanced Clinical Practice (Level 7) or equivalent
Independent Non Medical Prescriber (V300)
Evidence of ongoing CPD
Experience
Experience working in Urgent Care, Walk In Centre, Primary Care, or Emergency Department
Experience managing minor illness presentations autonomously
Experience prescribing independently
Experience using SystmOne and Adastra
