Main duties of the jobWe're excited to offer a fantastic opportunity for an enthusiastic and skilled Advanced Clinical Practitioner to join ourAgeing Well team - a service dedicated to helping adults with complex or multiple long term conditions, including frailty, stay well and independent in their own communities.
In this highly autonomous role, you will take the lead in assessing, managing and supporting individuals with complex needs, using a case management approach to keep conditions stable, prevent unnecessary hospital admissions, and improve overall quality of life. You will play a vital part in shaping and sustaining a service that truly makes a difference.
As a senior clinician, you will also champion our organisations values, ensure safe and high quality care aligned with CQC standards, and contribute to ongoing service development. If you thrive on innovation, collaborative working and driving positive change, this is a fantastic opportunity to make your mark.
