Advanced Clinical Practitioner

We are seeking a confident and dedicated Advanced Clinical Practitioner (ACP) to join our team.
This is a great opportunity for an ACP to deliver autonomous, high quality patient care in a General Practice setting, with the added benefit of support from our experienced team.
Main duties of the jobAssess, diagnose, plan, implement & evaluatetreatment & care for patients presenting with undifferentiated diagnosis including those with complex needs
Examine & assess patient needs from aphysiological and psychological perspective & plan clinical care
Proactively identify, diagnose & initiatetreatment plans for patients at risk of developing a long-term condition
Diagnose & manage acute &chronicconditions, integrating both drug- & non-drug-based treatment methods into amanagement plan
Prescribe & review medication for therapeuticeffectiveness, appropriate to patient needs & in accordance withevidence-based practice & national & practice protocols & within scopeof practice
Initiates appropriate diagnostic strategies & therapeutic interventions including laboratory request, ECGs with attention tosafety, cost invasiveness, acceptability & efficacy.
Interprets & evaluates diagnostic test results inliaison with the patient & their GP and initiates treatment accordingly
Work with patients in order to support compliancewith & adherence to prescribed treatments
Provide information & advice on prescribed orover-the-counter medication on medication regimens, side-effects andinteractions
Prioritise health problems, interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
Assess, identify & refer patients presenting withmental health needs in accordance with the practice guidelines for MentalHealth

You will be part of a team of six GP partners, three salaried GPs, Advanced Clinical Practitioners and a Nurse Practitioner, who work closely together to deliver excellent patient care.
We have an experienced and excellent team of Practice Nurses, HCAs, Phlebotomists and admin team.
We are supported by the Purbeck Primary Care Network team, which include First Contact Physiotherapists, Clinical Pharmacists, Mental Health Practitioners, Respiratory and Diabetes specialist support and Care Coordinators.
We are a training Practice and have a buddy system in place to help with continued support and learning and we offer ongoing training and CPD opportunities.

Job responsibilitiesThe postholder will be an experienced advanced clinical practitioner who, acting withintheir professional boundaries, will work autonomously to provide care forpatients presenting at the practice from initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care.
Theywill demonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the general practice inface to face and telephone appointments.
The postholder will demonstrate critical thinking in the clinical decision-makingprocess.
Theywill work collaboratively with the Duty Doctor and other clinical team members, tomeet the needs of patients, supporting the delivery of policy and procedures,and providing leadership as required.

Person Specification Experience- 2 years + NHS experience
- Experience of minor illness or acute care management
- Experience of working as part of an MDT
- Experience in examination of adults and children
- The ability to develop and maintain communication with people on complex matters, issues and ideas and/or in complex situations
- Ability to work autonomously using own initiative to manage workload and targets
- Sound ability to make clinical decisions
- Excellent problem solving skills
- Ability to manage and prioritise workload
- Competent and confident in using IT and computers
- Experience of working in a General Practice or Primary Care
- Experience of using SystmOne
- Experience of QOF
- Professional registration with the NMC or HCPC
- MSc Advanced Practice (post 2017) or ePortfolio route or working towards this.
- Qualified, registered independent non-medical prescriber or working towards this.
