We are seeking an experienced and enthusiastic Advanced Clinical Practitioner (ACP) to join our dynamic and supportive team at Satis House (B46 1TH) for 16 hours per week. At the heart of a modern and patient-focused general practice, you'll play a key role in delivering high-quality care to our local community.
As an ACP, you will work autonomously to assess, diagnose, treat, and manage patients presenting with a wide range of conditions. You'll be supported by a collaborative multidisciplinary team and benefit from a progressive environment that values innovation and continuous learning.
Main duties of the job- Provide expert clinical assessment and treatment to patients, both face-to-face and remotely
Manage acute presentations, chronic conditions, and minor illness independently
Prescribe medications safely and appropriately as an independent prescriber
Request, interpret and act on diagnostic tests and investigations
Refer patients to appropriate services or clinicians as needed
Contribute to chronic disease management, QOF achievement, and preventative health
Support the training and supervision of other clinical team members (if appropriate)
Maintain accurate and timely clinical records in line with practice policy

Job responsibilitiesConduct comprehensive assessments of patients presenting with acute, chronic, and complex conditions
Diagnose, treat, and manage patients independently, including prescribing medications as an independent prescriber
Order, interpret, and act on diagnostic tests and investigations
Manage caseloads effectively, prioritising urgent and routine care
Refer patients appropriately to secondary care or other services
Contribute to disease prevention and health promotion activities
Support QOF targets and chronic disease management programmes
Maintain accurate, up-to-date patient records in line with clinical governance standards
Participate in clinical audit, quality improvement, and research initiatives
Provide clinical leadership, mentorship, and support to nursing and administrative staff as appropriate
Engage in continuous professional development and maintain professional registration requirements
Person Specification Qualifications- Registered Nurse with current NMC registration
- Postgraduate qualification in Advanced Clinical Practice or equivalent
- Proven clinical experience in primary care or similar settings
- Strong clinical decision-making and communication skills
- Ability to work independently and collaboratively within a team
- Experience with EMIS and similar clinical systems
- Involvement in service development or leadership roles
- Knowledge of local healthcare pathways and community resources
