Job overview

HUC is seeking experienced Advanced Clinical Practitioners (Nurses and Paramedics with V300 Non-medical Prescribing) to join our urgent and unscheduled care services, delivering high-quality clinical care across both Out of Hours and Clinical Assessment Service pathways. This is an excellent opportunity for a highly skilled clinician to work autonomously within a dynamic and supportive multidisciplinary team, providing timely assessment, diagnosis, treatment, and ongoing management for patients with urgent and unscheduled healthcare needs.

The role offers a varied clinical caseload across telephone, remote, and face to face consultations, requiring advanced assessment, clinical reasoning, and decision making skills. You will play a key role in ensuring patients are directed to the most appropriate service and receive safe, effective, and person centred care. This position is well suited to an adaptable practitioner who is committed to clinical excellence, professional development, and contributing to the delivery of responsive urgent care services.

Main duties of the job
  • Undertake autonomous clinical assessments, diagnosis, treatment, and discharge of patients across both OOH and CAS services.
  • Manage a wide range of minor illnesses and injuries, including ENT and ophthalmic presentations, in line with evidence based practice.
  • Provide telephone, remote, and face to face consultations, including effective triage, streaming, and prioritisation according to clinical need.
  • Recognise and escalate acutely unwell patients, safeguarding concerns, or patients requiring urgent or emergency intervention.
  • Prescribe, review, and recommend treatment options in accordance with professional competence and local governance arrangements.
  • Offer clear, compassionate advice and health education to support self care and patient understanding.
  • Work collaboratively with the wider multidisciplinary team to deliver safe, efficient, and coordinated care, while maintaining accurate clinical records and contributing to audit, quality improvement, and service development.
