Advanced Clinical Practitioner - Digital Rectoscopy We have an opportunity for Advanced Clinical Practitioners to join our team on a fixed-term basis. The role is offered at 16 hours per week, which could either be taken up by 1 person doing 16 hours or 2 people doing 8 hours each.

This is an excitingopportunity to take a role as a key professional shaping a patient focusedservice, leading changes that will revolutionise how primary care services aredelivered.

You will be given autonomy andencouraged to use your clinical experience to work with patients, as well as workwith the Clinical Lead on how the service provides and demonstrates ahigh-quality service.

Main duties of the job This is an excellent opportunity for an experienced ACP who either has experience in the gastroenterology or would be excited to be trained. All candidates will receiving full training in using digital rectoscopy equipment led by national experts in the field.

The postholder will leaddigital rectoscopy clinics undertaking examinations in primary care on thosewith uncomplicated rectal bleeding aged under 50 years old. The technologyallows for video capture so that the ACP can be support by GPSI and Gastro consultantsas required.

We are looking for someone who is a registered healthprofessional with a prescribing qualification, passionate about enabling changewhich will positively impact primary care service users. You will need to havethe confidence and capability to be a leader in a field which is emergingnationally.

Job responsibilities Key Areas of Responsibility Clinical

1. Work autonomously as an advanced practitioner within thespecialty, managing a caseload of patients delivering individualised directpatient care.

2. Direct responsibility for assessment, examination,investigation and diagnosis of patients within their area of work.

3. Appropriately treat patients, resulting in the safemanagement and appropriate referral or care planning of patients withundifferentiated and undiagnosed presentations.

4. Manage own caseload and clinical priorities according toagreed protocols and working practices.

5. Undertake a variety of clinical skills and providetreatment/advice as per speciality and scope of practice. Using agreedprotocols of clinical practice and professional guidelines.

6. Within scope of practice and clinical competence requestand/or undertake diagnostic procedures and clinical investigations related toplans of care.

7. Utilise scope of practice to undertake Non MedicalPrescribing role and provide advice on medicine management issues associatedwith the patient specialty group.

8. Provide a seamless, high quality service from assessmentto diagnosis, treatment and review, referring to other specialists as required.Provide guidance to staff, patients and their families and carers on pathwaynavigation.

9. Lead in the development and updating of referralguidelines and policies for the service.

10. Use highly developed communication skills to effectivelycommunicate with colleagues, patients and their relatives/carers, makingreports and liaising as required with medical staff and other members of themultidisciplinary team both verbally and in writing.

11. Use professional judgement to act as an advocate forpatients to ensure a patient focused approach to the delivery of care. Supportand enable patients and carers to make informed decisions relating to theirtreatment and management. Escalate any concerns via clinical structures asrequired.

12. Work towards safe and timely provision of care ofpatients between Primary and Secondary Care services/healthcare professionals,ensuring barriers to care provision are identified and acted uponappropriately.

13. Report and raise concerns related to Safeguarding,accessing advice and support as required. Be conversant with Organisationalpolicies for safeguarding including the assessment of mental Capacity.

14. Ensure effective and accurate communication whetherverbal or written between healthcare professionals as necessary.

15. Ensure that high standards of all documentation aremaintained, with accurate, complete and up-to-date information regardingpatient care are kept in accordance with professional standards.

16. Undertake training where necessary to develop furtheradvanced clinical practice roles required by the service in order to provide ahigh standard of patient care.

17. Practice within the scope of professional registration(i.e. NMC, HCPC).

Professional Leadership & Management

1. Provide highly visible and accessible professionalleadership and demonstrate expert knowledge and standards of clinical practice.

2. Lead and develop a defined area of Advanced Clinicalpractice within the designated area of practice, promoting interdisciplinaryteam and collaborative working practices.

3. Act as a clinical role model demonstrating high standardsof advanced clinical practice and provide support or advice to other staff whennecessary

4. Lead and support the development of the role according tochanging patients needs, service requirements and evidence base practice.

5. Lead and actively participate in service/departmentalprojects, quality initiatives and statutory accreditation processes. This willinclude the setting and monitoring of clinical standards of care.

6. Ensure that all resources are managed in an efficient andcost-effective manner to achieve maximum effect for patients and staff.

7. Adhere to all relevant Organisational policies andprocedures and to ensure that they are correctly implemented.

8. Lead in the implementation of multidisciplinary serviceobjectives that reflect Organisational or PCN strategies for patient care.

9. Demonstrate effective leadership skills, supporting the Clinicalmanagement team in service development and lead as delegated on theimplementation of any of these changes.

10. Monitor standards and maintain high quality care. Reportany clinical incidents promptly via the Organisational pathways.

11. Utilise the highest level of interpersonal andcommunication skills when dealing with complex, sensitive or emergencysituations.

12. Maintain an awareness of professional and ethical issuesto ensure care is delivered in a professional timely and courteous manner byall members of the team, respecting the different spiritual and culturalbackgrounds of colleagues, patients and relatives.

13. Escalate any concerns or complaints promptly to ClinicalLead and take part in any necessary Clinical Governance.

14. Participate in the recruitment process as required,providing feedback and support in recruitment decisions.

15. Accountable for own practice, acting at all times withinthe Professional Code of Professional Conduct (NMC,HCPC, CSP)

16. Maintain a working knowledge of local and nationalprofessional policy and strategy.

Education & Training

1. Act as an expert educational resource for clinical staff,patients, and carers by providing formal and informal education. Promote anddevelop a positive learning environment throughout the Organisation.

2. Act as mentor/assessor to staff members and students asrequired, providing educational advice and support. Ensure other clinicians canaccess appropriate training support and guidance.

3. Lead and actively participate in the delivery ofeducational programmes for multi-disciplinary clinicians.

4. Provide support and guidance as required to cliniciansfrom all disciplines.

5. Ensure that pre and post registration students receiveappropriate learning experience whilst allocated to the Organisation/PCN.

6. Participate in provision and identification of in-servicetraining need for all team staff.

7. Participate in education and practice development on a PCNwide basis liaising with PCN, PCS or External educational leads to ensureeducational objectives are delivered.

8. Recognise the limits of own professional practice andcompetence, undertake further training and academic qualifications as requiredto maintain own specialist knowledge.

9. Take responsibility for ensuring personal and completionof any statutory or mandatory training as required for. Informing line managerif there is any deviance from training attendance.

10. Adapt clinical knowledge and skills to differentclinical settings.

11. Participate in the annual appraisal process, delegatingduties to team members as appropriate to ensure all staff within the team havepersonal development plans which support revalidation.

12. Maintain close links with local HEIs and participate inin the development and delivery of new and established advanced practiceprogrammes as required.

Research & Audit

1 Utilise research findings and audit in the delivery ofadvanced clinical patient care, developing new ways of working.
