Job Description

Our Client, a Welsh council is seeking an Adult's Social Worker to join their Single Point of Access Team.

Responsibilities of this role:

  • Act as the first point of contact for Adult Social Care, screening and triaging referrals from professionals, service users, carers, and partner agencies.
  • Undertake strengths-based assessments to identify needs, risks, outcomes, and eligibility under the Care Act 2014.
  • Identify and respond to safeguarding concerns, completing initial enquiries and making appropriate referrals in line with safeguarding procedures.
  • Provide information, advice, and signposting to community resources, preventative services, and support networks to promote independence and wellbeing.
  • Carry out risk assessments and implement immediate safeguarding or support measures where urgent intervention is required.
  • Work collaboratively with health services, housing, police, voluntary organisations, and internal teams to coordinate effective support and timely interventions.
  • Maintain accurate case records, assessments, and decision-making documentation in accordance with legislative requirements and organisational policies.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have minimum of 3 years post qualifying social work experience
