Adult's Social Worker - Neighbourhood Team

Our Client, Bury Council, is seeking an Adults Social Worker to join their Team.

Responsibilities of this role:

• Hold longer-term adult cases within a local community area
• Complete Care Act assessments and reviews
• Develop and monitor care and support plans
• Safeguard adults at risk (s42 enquiries)
• Work with carers and complete carers' assessments
• Support independence, wellbeing, and prevention
• Coordinate multi-agency and community-based services
• Review commissioned care packages and outcomes
• Maintain clear, timely, and lawful records

Requirements of this role:

• Must be a Qualified Social Worker, registered with Social Work England.
• Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
• Must have minimum of 3 years' experience post qualifying working as a Qualified social worker
