Hello - we are looking for an experienced ACP/Specialist Paramedic or Nurse Practitioner to join our team.

We have a great team that work across our collaborative and supportive network of 4 practices providing a much valued Home Visiting service for housebound patients, as well as practice based clinics.

We are looking to fill a full-time position (37.5 hours per week), but would also consider 2 x part-time roles.

A typical day consists of:

A practice based clinic for the morning of face to face and telephone consultations followed by home visits (that have been triaged by the practice)

Minerva Primary Care Network (PCN) is a group of 4 GP practices located in Bath consisting of Combe Down Surgery, Newbridge Surgery, Rush Hill & Weston Surgery and St Michael's Surgery. We have a combined list size of 40,000 patients and we continue to grow.

We have a strong and successful history of working collaboratively and have a number of staff successfully working across all our practices. Collectively we have a strong focus on high quality patient care.
