Job Description
3 x Specialty Doctors in Geriatrics - Perm or FTC - Essex

Triple West Medical are currently seeking multiple experienced Specialty Doctors in Geriatrics for a FTC post for 12 months minimum.

The Trust have over 10,000 staff offering comprehensive range of acute and community services over a number of sites to over 1 million people. The NHS Trust wish to continue to build their reputation as well as advancing standards of clinical care that are at the cutting edge of their specialty. They are looking for applicants who will contribute towards this and who will help to drive the development of modern emergency care in the local health community.

The frailty assessment unit is a 12 bedded ambulatory unit that enables older patients to receive holistic and comprehensive geriatric assessment. The unit receives patients from both the 'front door' and also community referrals.

They have excellent working relationships with community teams and General practitioners who can refer to the unit directly and also receive patients from A&E and clinical decision unit.

Multidisciplinary rapid comprehensive Geriatric Assessment (CGA) methodology is employed in the Frailty Assessment unit and the service is Consultant led.

They also operate ambulatory 'hot' clinics within the unit. The frailty team has a daily presence in ED focusing on admission avoidance and works closely with the ED and Acute Medical colleagues.

The unit is a highly performing service with true MDT working and is well recognised across the Trust. GPs have direct access to a Consultant Geriatrician 5 days a week for phone advice and access to ambulatory Frailty and 'Hot clinic' slots.

Responsibilities:
  • Deliver a dedicated programme of geriatric care
  • Assume responsibility for patients on Elderly Medicine wards with daily review and support to consultant ward rounds
  • Regular pro-active review of patients and coordination of care with allied specialties
  • Engage in multi-disciplinary meetings to direct the care of patients
  • Provide specialist opinions to other hospital specialties as part of frailty liaison service
  • Participate in outpatient clinics
  • Support the delivery of patient care within Geriatric Medicine in line with national standards and Care Quality Commission requirements
  • Actively participate in developing innovative approach to acute elderly care
  • Develop older peoples acute service pathways
  • Develop older people services / Outpatient clinic
  • Facilitate the further development of the Frailty Assessment Unit
  • Actively participate in clinical audit and quality improvement initiatives
