We have an exciting opportunity for a Service Team Manager to lead our Hospital Discharge Team!
Main duties of the jobAs Service Team Manager for the Hospital Discharge Team, you will lead a multidisciplinary team working across acute settings to ensure safe and person-centred discharges. You will:
- Provide visible, supportive leadership to staff, promoting resilience and reflective practice.
- Coordinate effectively across health and social care partners to advocate for individuals' rights and needs.
- Ensure compliance with statutory duties including the Care Act, Mental Capacity Act, Continuing Health Care, and safeguarding.
- Champion strengths based and person-centred practice in a fast paced, multidisciplinary environment.
- Contribute to RBWM's strategic priorities: building inclusive, healthy communities, promoting prevention and choice, and delivering compassionate, high quality services.

Job responsibilitiesWe are recruiting a knowledgeable and compassionate Service Team Manager to lead our Hospital Discharge Team at the Royal Borough of Windsor & Maidenhead. This important role is central to delivering our Social Care Futures vision and ensuring that hospital discharges are safe, timely, and person-centred.
This is a full-time, permanent role with hybrid working, and a salary of £53,296 - £58,286 per annum.
Person Specification Qualifications- Be a professionally qualified Social Worker holding either CQSW, DiPSW, Degree in Social Work or equivalent.
- Registered with Social Work England.
- PEPS2 qualified and experienced practice educator (desirable).
- Qualified as a Best Interest Assessor (desirable).
- Substantial experience in adult social care (minimum 3 to 5 years), ideally including roles as a Senior Practitioner or Assistant Team Manager or Team Manager.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure & Barring Service (formerly known as CRB) to check for any previous criminal convictions.
