Case Manager, Long Term Accommodation, Mid West

Location: Limerick

Contract: Full Time, Specific Purpose

The purpose of this role is to work as a member of the Housing Support Services Team providing housing support and management for customers accessing Focus Ireland Housing Services in Limerick.

What you will do:
  • Work within the overall objectives of Focus Ireland as part of the Support Team based in Limerick, under the supervision of the Project Leader.
  • Manage a caseload of clients, providing one-to-one support across areas such as independent living skills, addiction, budgeting, and mental health.
  • Liaise with internal teams and external agencies, making referrals and coordinating supports to meet client needs and promote independent living.
  • Build and maintain effective working relationships with statutory and voluntary agencies.
  • Maintain accurate records and documentation in line with Focus Ireland policies and procedures.
  • Prepare written reports and updates as required.
  • Support residents in maintaining their accommodation to an appropriate standard, including responsibility for reporting repairs.
  • Work collaboratively with other Focus Ireland departments, including HR, finance, services, and development teams.
  • Use internal systems such as ORION and MIP to record and manage client information.
  • Engage effectively with colleagues across the organisation to support service delivery.
  • Attend team meetings, training, supervision sessions, and contribute to ongoing professional development.
  • Undertake lone working in the community in line with policy and safety procedures, and carry out additional duties as assigned by the Project Leader.
What you will need:
  • A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology etc and/or be a CORU registered Social Care Worker.
  • Extensive experience working in a social care setting.
  • Group work experience, particularly facilitation.
  • Key-working experience.
  • Advocacy and liaising with other agencies.
  • Ability to cope with and manage lone working including out of hours work.
  • Full driver's license with access to a car.
