Job Description

This role will provide a hybrid service that will encompass a generic Care Co-ordinator for the Practice as well as offering wider practice administration support.

Care coordinators play an important role to proactively identify and work with people to provide coordination and navigation of care and support across health and care services. They work closely with the Practices multi-disciplinary teams, acting as a central point of contact to ensure appropriate support is made available to patients and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.

The successful candidate will be working across Newport Health Centre registered sites. They will be a caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible. attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.

Main duties of the job

Support patient and carers with health needs as identified.Further administrative support will be provided to the wider practice and management team to ensure the smooth running of services and day to day operations.

  • Experience of working with the general public
  • Experience of administrative duties
  • Experience of working in Primary Care
  • Knowledge of S1 clinical system
  • Experience of coordinating services, people and processes
  • Experience of writing and promoting standard operating procedures
  • Experience of promoting campaigns and knowledge of NHS communication platforms
  • Knowledge of the needs of vulnerable people, safeguarding and the associated legislative framework
  • Experience of developing relationships with a wide variety of people
Qualifications
  • Educated to GCSE level or equivalent
  • GCSE Mathematics & English (C or above)
