Advanced Clinical Practitioner

An exciting opportunity has arisen for an experienced and enthusiastic Advanced Clinical Practitioner (ACP) to join the Older Adult Community Mental Health Team within Midlands Partnership University NHS Foundation Trust (MPFT), covering Shropshire, Telford and Wrekin.

You will play a key role in the assessment, diagnosis and management of older adults presenting with complex mental health conditions, including dementia and functional disorders, working autonomously and collaboratively as part of a multidisciplinary team.

You will utilise advanced clinical reasoning, leadership, and decision making skills to deliver high quality, person centred care in the community. The role also involves supporting service improvement, clinical education and supervision of colleagues to ensure safe and effective service delivery.

Main duties of the job
  • Undertake comprehensive assessments of service users with complex mental health needs, utilising advanced clinical reasoning to inform formulation and treatment plans.
  • Exercise autonomous clinical decision making within professional and Trust frameworks.
  • Prescribe and monitor treatment in accordance with professional competencies and Trust governance.
  • Lead and contribute to multidisciplinary discussions, risk assessments, and care planning to ensure safe and effective care coordination.
  • Provide expert clinical advice, leadership, and supervision to support team members and students.
  • Work in partnership with service users, carers, GPs, primary care networks, social care, and voluntary agencies to deliver integrated and recovery focused care.
  • Contribute to the development and implementation of evidence based practice, quality improvement projects, and service development initiatives.
  • Participate in audit, evaluation, and research activities to inform best practice.

Person Specification Qualifications
  • Relevant Master level qualification as an ACP
  • Professional regulated core qualification with the NMC/HCPC
  • Independent and supplementary prescribing qualification
  • Mental Health Practitioner
  • Qualifications in Project Management and/or Quality improvement
  • Mentorship/ teaching qualification
Knowledge / Skills.
  • Extensive knowledge and application in practice of leading MDTs, professional meetings, MHA
  • A problem solving approach to professional practice
  • Demonstrate values that are complementary to those of the organisation service
Experience
  • Experience in assessing, formulating and managing risk for people with severe and enduring mental health difficulties
  • Experience of engaging in service development activities
  • Evidence of promoting research, evidence based practice, clinical excellence and quality approaches to support outcome focussed care
  • Evidence of working effectively at a senior level in a multi disciplinary health and social care setting (mental health)
  • Experience of working therapeutically in community settings
  • Skills related to Early Signs Monitoring and relapse management
